About The Position
Comfort Home Care is seeking a Registered Nurse (RN) for a full-time opening in the Lowell area, focusing on psychiatric and home health patients. This role emphasizes work-life balance, allowing nurses to build meaningful relationships with patients and provide compassionate care in their homes. The position offers flexibility, autonomy, and support, enabling nurses to help patients remain independent and avoid unnecessary hospitalizations. The role involves managing one's own schedule, making clinical decisions, assessing patients, creating and modifying care plans, educating patients and families, communicating with care teams, and documenting visits from home.
Requirements
- Active Registered Nurse (RN) license in Massachusetts.
- One year of nursing experience.
- A drive to provide compassionate, patient-centered care.
- Reliable transportation for patient visits.
Nice To Haves
- Home health or psych nursing experience preferred.
Responsibilities
- Assess and adapt to each patient's current condition, medical needs, and overall well-being during visits.
- Create and modify patient care plans based on individual needs and changing conditions.
- Teach and support patients, providing medication guidance or wound care education.
- Communicate and coordinate with providers, families, and care teams to ensure timely and appropriate care.
- Document patient visits accurately from home for compliance and continuity of care.
