We are seeking a compassionate and skilled Registered Nurse (RN) to join our team and serve patients from Imperial Beach to Santee. This role involves planning, organizing, and coordinating home care services, with a strong emphasis on community health and patient education.
Requirements
- Active California RN license
- Minimum 1 year of direct patient care experience (home health preferred)
- Current CPR certification
- Valid driver’s license and insured, reliable vehicle

Responsibilities
- Develop and initiate individualized care plans in collaboration with patients and caregivers
- Deliver physician-ordered nursing services and implement preventive and rehabilitative procedures
- Educate patients and caregivers to promote health and independence
- Identify and implement discharge planning needs
- Coordinate necessary equipment and services
- Supervise LPNs, LVNs, and HHAs
