About The Position
Concierge Home Health Registered Nurse (RN) needed for TheKey Concierge Nursing Services to serve clients across Los Angeles and Orange County. This role involves independence, flexibility, and premium patient care. Primary opportunities include in-person home visits, initial assessments, and quality assurance re-evaluations, with future expansion into shift-based work. The nurse will be called on an as-needed basis and priority is given to those open to last-minute requests.
Requirements
- Active, unencumbered Registered Nurse (RN) license in California
- Minimum 2 years of nursing experience
- Flexible schedule with willingness to work on-call, per diem, and accept last-minute visit assignments.
- Valid California Driver’s License, reliable vehicle, and active auto insurance.
- Strong clinical assessment capabilities, exceptional customer service, direct conflict resolution skills, and tech-savviness for timely EHR documentation.
Nice To Haves
- BSN preferred
- Background in home health, elder care, or care management strongly preferred.
Responsibilities
- Conduct in-person initial nursing assessments, reassessments, change-of-condition evaluations, and home safety reviews to build tailored, high-touch Care Plans.
- Perform skilled nursing visits, routine wellness checks, and preventative care in accordance with physician orders and individual care plans.
- Accept per diem visit assignments on an as-needed basis, including responsiveness to last-minute patient requests or coverage needs.
- Serve as the main clinical point of contact, building strong rapport with clients, family members, and multidisciplinary care teams.
- Complete and submit accurate clinical notes in a timely manner via our electronic system, ensuring strict compliance with state home health regulations.
- Transition seamlessly into block shift-work opportunities as case volumes and care needs expand in the region.
