About The Position
Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health Registered Nurse to provide skilled nursing care for patients in their homes throughout the Gainesville and Ocala area. The Schedule for the RN PRN is 5-10+ visits per week and some weekend/holiday on-call rotations. Requirements
- Graduate of an approved nursing program with an active Florida RN license
- Requires home health RN experience.
- Strong assessment, critical thinking, and patient education skills
- Willingness to participate in weekend and holiday on-call rotations
Nice To Haves
- BSN or MSN degree
- Experience using Homecare Homebase (HCHB) EMR
Responsibilities
- Perform comprehensive patient assessments, including OASIS assessments at required intervals
- Develop, initiate, and revise individualized plans of care as patient needs evolve
- Deliver skilled nursing services and treatments as outlined in the plan of care
- Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
- Collaborate with the interdisciplinary team to ensure seamless, holistic care
