About The Position

A Passionate Helping Hand Home Health Care is committed to providing compassionate, high-quality in-home healthcare services to patients in need. We believe in a patient-centered approach, ensuring individuals receive the care, support, and medical attention they deserve in the comfort of their homes. We are seeking a skilled and dedicated Registered Nurse (RN) to join our team and provide professional nursing care to patients in a home health setting. The RN will work closely with patients, families, and interdisciplinary healthcare teams to develop and implement personalized care plans, monitor patient progress, and ensure the highest level of care is delivered.

Requirements

  • Active RN license in Indiana.
  • Minimum of one year of nursing experience (home health experience preferred).
  • CPR/BLS certification required.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to work independently and manage multiple patients efficiently.
  • Compassionate, patient-centered approach to care.
  • Valid driver’s license and reliable transportation for home visits.

Nice To Haves

  • Home health experience preferred.
  • Experience with electronic medical records (EMR) preferred.

Responsibilities

  • Conduct in-home patient assessments and develop individualized care plans.
  • Provide skilled nursing care, including wound care, medication management, IV therapy, and injections.
  • Monitor patient conditions, document changes, and communicate concerns to physicians and care teams.
  • Educate patients and caregivers on disease management, medication adherence, and lifestyle modifications.
  • Collaborate with physical therapists, occupational therapists, social workers, and home health aides to coordinate care.
  • Ensure compliance with all state, federal, and agency regulations and protocols.
  • Maintain accurate and up-to-date patient records and documentation.
  • Provide emotional support and guidance to patients and their families.
  • Participate in case conferences and team meetings to discuss patient care plans.
  • Promote a safe and healthy home environment for patients.
