About The Position

The Registered Nurse (RN) will be the clinical anchor for home-based care, conducting in-home patient assessments and ongoing evaluations, developing and updating individualized care plans, and supervising and supporting CNAs, GNAs, HHAs, and Med Techs. This role involves monitoring changes in patient condition, coordinating follow-up care, communicating with families, physicians, and care teams, ensuring safe and compliant care delivery, and maintaining accurate clinical documentation. The RN will also provide guidance, education, and clinical support to caregivers, directly shaping patient outcomes and quality of life.

Requirements

• Active Maryland RN license (or compact license eligible in MD)
• CPR/First Aid certification
• Reliable transportation for home visits
• Strong clinical assessment and communication skills

Nice To Haves

• Experience in home care, hospice, or geriatrics preferred

Responsibilities
