Registered Nurse (RN) Home Care - Part Time
About The Position
OCES is growing and expanding its services to 11 more towns covering the south shore area. OCES supports older adults and individuals with disabilities by providing information and coordination of services promoting healthy, safe lives for our consumers, helping them stay active and engaged in their lives and communities. OCES is seeking a Registered Nurse for its Home Care Department to assess and/or authorize services delivered by provider agencies to support consumers in the community. This role will also be cross-trained to facilitate successful discharge activities and other transition support activities for the Transition Support Team.
Requirements
- Current Massachusetts RN License
- Two years' clinical/community-based experience.
- Extensive knowledge of the medical, emotional and social needs of elders and people living with disabilities.
- Excellent verbal and written communication skills.
- Proficient with Microsoft Office Suite or related software.
- Considerable knowledge of developing assessment care plans.
Responsibilities
- Determine consumers' eligibility for programs and services according to procedures and guidelines.
- Establish, review and revise care plans.
- Work with the care management team to determine appropriate service options; promote cost-effective service substitutions; establish the frequency, scope and duration of services; and jointly authorize service packages.
- Maintain knowledge of current OCES policies and standards, and funding source regulations.
- Providing consultation to providers and other community agencies.
- Conduct and/or participate in training and workshops as needed.
- Provide consultation to HC Program staff.
