About The Position

OCES is growing and expanding its services to 11 more towns covering the south shore area, including Quincy, Braintree, and Weymouth. OCES supports older adults and individuals with disabilities by providing vital information and coordination of services. Our mission is to promote healthy, safe living, allowing consumers to stay active and engaged in their lives and communities. We are seeking a dedicated and caring Registered Nurse for our Home Care Program. This role will be responsible for assessing and authorizing services delivered by provider agencies, aiming to promote coordination and cooperation for cost-effective, quality services.

Requirements

  • Excellent verbal and written communication skills
  • Excellent interpersonal and conflict resolution skills
  • Excellent organizational skills and attention to detail
  • Strong analytical and problem-solving skills
  • Proficient with Microsoft Office Suite or related software
  • Considerable knowledge of developing assessment care plans
  • Extensive knowledge of the medical, emotional and social needs of elders and people living with disabilities
  • Associate degree in Nursing or related field
  • Current Massachusetts RN License
  • Two years' clinical/community-based experience
  • Must have personal computer with secure internet access at home

Responsibilities

  • Determining consumers' eligibility for programs and services according to procedures and guidelines
  • Establishing, reviewing and revising care plans in accordance with funding source regulations, OCES procedures and standards
  • Determining appropriateness for specific service options; promoting cost-effective service substitutions; establishing the frequency, scope and duration of services; and jointly authorizing appropriate service packages
  • Maintaining records and preparing reports as required
  • Maintaining knowledge of current OCES policies and standards, and funding source regulations
  • Performing role of Lead Assessment Coordinator; participate in interdisciplinary
  • Providing consultation to providers and other community agencies
  • Conduct and/or participate in training and workshops as needed
  • Provide consultation to HC Program staff
  • Other duties as assigned
