Registered Nurse (RN) - Full-Time On-Call Community Hospice

About The Position
The Community Hospice is seeking a compassionate and dedicated Full-Time Registered Nurse (RN) to join our After-Hours Hospice Care Team. In this rewarding role, you will provide critical nursing care and support to hospice patients and families in their homes and nursing facilities throughout the Capital Region and Saratoga area during evening and overnight hours. Schedule - Full-time evening or overnight shifts. Every other weekend commitment required.
Requirements
- Valid NYS RN license required.
- 1-2 years previous RN experience required
- Strong assessment, communication, and critical thinking skills.
- Ability to learn/use an electronic medical record
- Clean Driver's License and reliable vehicle
Nice To Haves
- homecare and/or Hospice experience preferred.
- Hospice experience is preferred but not required.
Responsibilities
- Provide after-hours nursing visits to hospice patients in home and facility settings
- Assess and manage symptoms, addressing urgent patient and family needs.
- Provide education, guidance, and emotional support to patients and caregivers.
- Coordinate care with the interdisciplinary hospice team.
- Communicate changes in patient condition and ensure appropriate follow-up.
- Maintain accurate and timely clinical documentation.
- Work with interdisciplinary team.
