About The Position

The Home Health Resource Visit Nurse is a crucial role within the organization, acting as a reliable support for various home health teams. This position involves providing exceptional patient care across different territories in the Twin Cities Metro area, helping to manage increased patient census, cover for clinician vacations, or assist when other clinicians need support. Requirements

  • Active Minnesota Registered Nurse (RN) license
  • Experience in home health, hospital, rehabilitation, skilled nursing, or acute care
  • Strong clinical judgment and assessment skills
  • Comfortable adapting to different territories and patient populations
  • Excellent communication and organizational abilities
  • Reliable transportation and willingness to travel throughout the Twin Cities Metro

Responsibilities

  • Provide skilled home health nursing visits throughout multiple service areas
  • Perform comprehensive patient assessments
  • Deliver wound care, medication management, injections, and other skilled nursing services
  • Educate patients and caregivers on diagnoses and treatment plans
  • Collaborate with physicians, therapists, and interdisciplinary care teams
  • Float between territories to support census growth, vacations, and staffing needs
  • Complete timely documentation using Homecare Homebase (HCHB)
  • Help ensure continuity of care for patients across the organization
