About The Position

Electrophysiology (EP) Nurse Navigator - Seeking nurses with cardiac, procedural, electrophysiology, critical care, cath lab, telemetry, or related cardiovascular experience who will be responsible for assisting patients, families, and providers through the continuum of electrophysiology care to resolution of treatment. Ensures timely access, coordination, and continuity of care for patients requiring electrophysiology evaluation and procedures. Serves as a clinical resource for patients undergoing EP interventions, promoting optimal outcomes and patient experience. Requirements

  • Current Michigan Nursing License
  • Graduation from accredited nursing program: BSN Required.
  • Three or more years of progressive clinical nursing experience in cardiovascular health.

Responsibilities

  • Coordinates care for patients referred for electrophysiology procedures, from referral through post-procedure follow-up.
  • Identifies appropriate ablation candidates and partners with providers to ensure timely evaluation and treatment.
  • Coordinates required testing, consultations, and clinical clearances to support procedural readiness.
  • Oversee pre-procedural preparation, including medication management, patient education, and readiness review to reduce delays and cancellations.
  • Serves as the primary liaison between patients, providers, and procedural teams to ensure seamless care coordination.
  • Monitors patient progression through the EP care pathway, proactively addressing barriers to treatment and access.
  • Supports program growth and operational performance through improved patient preparedness, procedural efficiency, and quality outcomes.
  • Coordination of information and patient transfer to other hospitals that have availability to do valve procedures.
  • Provides and/or facilitates clinical and emotional support, education and resources to patients and their families.
  • Works with patients, providers and caregivers to identify possible barriers to care and systematically helps to break down barriers (i.e. Financial, transportation, language, etc.).
  • Facilitates as necessary, timeliness of appointments; works continuously to reduce the intervals between time of abnormal results, diagnosis and treatment.
  • Develop and maintains relationships with providers, internal service delivery departments, other caregivers and patients in order to coordinate services within the organization, at outside facilities and within the community.
  • Assists patients in accessing support and follow-up services.
  • Maintains contact with patients throughout the continuum of care; meets patients at their doctor’s appointments as necessary; addresses questions and concerns.
