The Home Therapies Program offers paid, one-on-one training and education, empowering and engaging dialysis patients in peritoneal and home hemodialysis using state-of-the-art programs and equipment. This role primarily functions in a clinic setting, with initial home visits conducted alongside another staff member. The position serves as an onsite Patient Educator and Case Manager for patients with various shifts and work hours. The professional registered nurse Home Therapies RN CAP 2 may be an entry-level designation into the Clinical Advancement Program (CAP) for new employees meeting specific criteria or through advancement from RN CAP 1. This role is accountable for the provision and coordination of clinically competent care, including assessment, planning, intervention, and evaluation for an assigned patient group. This may involve delegating tasks to direct patient care staff. As part of the End Stage Kidney Disease (ESKD) health care team, this position contributes to decision-making, teaching, leadership, and quality improvement activities to enhance patient care outcomes and facility operations.
