About The Position

The Community Hospice has a wonderful opportunity for a full-time RN to work as part of our Hospice Home Care Team caring for patients and families residing in Saratoga/Washington/Montgomery county areas. This role involves providing compassionate end-of-life care and support to patients and families in the comfort of their homes. The position is ideal for someone who enjoys working closely with a team, which includes Aides, LPNs, medical directors and NPs with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers. The Community Hospice offers comprehensive training and support, including a thorough orientation program, specialized training in end-of-life and hospice care, dedicated preceptors, and ongoing mentorship. They prioritize safety by providing comprehensive training and necessary protective equipment. This role offers a Monday to Friday work schedule with no weekends or holidays, contributing to work-life balance.

Requirements

A current license to practice as a Registered Nurse in the State of New York
Associates Degree in Nursing, BSN preferred
1 year experience as an RN required-preferably in Hospice/Home Care or acute care.
Valid Driver's License and Reliable vehicle
The RN must be able to communicate effectively, verbally and written
High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers

Nice To Haves

Understanding of Hospice principles preferred
BSN preferred

Responsibilities

Coordinate with an interdisciplinary team, to develop a comprehensive plan of care to provide added support to patients and families in their homes.
Provide compassionate end-of-life care and support to patients and families in the comfort of their homes.
