Registered Nurse (RN) - Community Hospice Per Diem Columbia/Greene Counties
About The Position
Come join St. Peter's Health Partners Community Hospice in the Catskill Region. This is a Per Diem RN position for Community Hospice Homecare, serving the Columbia and Greene County areas. The role involves caring for patients and families residing primarily in Columbia and Greene counties, with needs during the week, Monday to Friday, from 8 am to 4:30 pm. No weekend needs are expected. The Community Hospice offers a strong orientation program, training in End of Life Care, and mileage reimbursement. This role is ideal for someone who enjoys working closely with an interdisciplinary team, including Aides, LPNs, medical directors, NPs, social workers, chaplains, complementary therapists, and volunteers, to develop comprehensive care plans.
Requirements
- A current license to practice as a Registered Nurse in the State of New York
- Associate's degree in nursing, BSN preferred
- 1 year experience as an RN required, preferably in Hospice/Home Care or acute care.
- Valid Driver's License and Reliable vehicle
- The RN must be able to communicate effectively, verbally and written
- High level of interpersonal skills to establish and maintain relationships with patients, families, physicians, and coworkers
Nice To Haves
- Understanding of Hospice principles preferred
Responsibilities
- Coordinate with an interdisciplinary team to develop a comprehensive plan of care.
- Provide added support to patients and families.
