About The Position

The Community Hospice is seeking a compassionate and motivated Registered Nurse (RN) to join our Weekend Hospice Admissions Team. This rewarding role focuses primarily on admissions to our Hospice Home Care program, with cross-training opportunities to support admissions across other levels of hospice care. This is a full-time position with a weekend shift, working only Saturdays and Sundays from 8 AM to 8 PM. Once trained, no hours are required during the week. The role involves evaluating and admitting patients to hospice services in the home setting, ensuring a smooth transition into care. It combines clinical expertise, compassionate communication, and coordination with physicians, patients, families, and the hospice interdisciplinary team. The service area covers eight counties: Albany, Columbia, Greene, Rensselaer, Saratoga, Schenectady, Washington, and Montgomery. Patient assignments are made with consideration for the geographic region.

Requirements

  • Valid NYS RN license
  • Associate’s degree in nursing.
  • 1 year of RN experience is required.
  • Valid driver license in good standing and reliable vehicle
  • Strong communication and interpersonal skills.
  • Ability to document clinical information concisely and accurately.
  • Strong computer proficiency.
  • Excellent organizational and time management skills.
  • Must be able to communicate effectively with patients, physicians, and peers.

Nice To Haves

  • BSN Preferred
  • At least 1 year experience as an RN working in homecare highly preferable.
  • Previous hospice experience preferred but NOT required.

Responsibilities

  • Conduct hospice admissions for patients in their homes, averaging two home admissions per day.
  • Collaborate with community physicians and Hospice Medical Directors to confirm hospice eligibility.
  • Complete comprehensive assessments of patient and family needs.
  • Initiate the plan of care, including coordination of medications, durable medical equipment, medical supplies, and interdisciplinary support services.
  • Coordinate care with social workers, chaplains, volunteers, complementary therapists, home health aides, and other team members.
  • Ensure a seamless handoff to the assigned Hospice Case Manager.
  • Accurately document assessments, care plans, and admission-related information.
