About The Position

The Community Hospice has a rewarding opportunity for a full-time RN to work as a part of our Nursing home/Admissions Team across our catchment areas of 9 counties Monday - Friday 8a-4:30p. The Hospice Admissions RN admits patients to hospice services. The RN coordinates with the patient's community/nursing home physician and our hospice medical directors to confirm eligibility for hospice services. The workday includes two admissions to Hospice. Go to nursing homes to complete the initial assessment of patient/family needs and begin implementation including ordering medications, medical equipment, supplies and support from our interdisciplinary team which includes social workers, chaplains, volunteers, complementary therapists, home health aides, and more! Ensure a smooth hand-off to the Hospice Case Manager. Corporate office is 445 New Karner Road-Albany NY with satellite offices in Catskill and Saratoga NY. Admissions are across 9 counties: Columbia, Greene, Albany, Rensselaer, Schenectady, Saratoga, Washington, Schoharie. Your region is considered in patient assignment.

Requirements

  • A current license to practice as a Registered Nurse in the State of New York
  • Associates Degree in Nursing, BSN preferred
  • 1 year experience as an RN required.
  • Valid Driver's License and Reliable vehicle
  • Excellent communication skills. Must be able to communicate effectively with patients, physicians, and peers
  • Must be able to document key information concisely and accurately
  • Strong computer skills
  • Strong organizational and time management skills

Nice To Haves

  • BSN preferred
  • Long term care experience or homecare experience is highly preferred.
  • Hospice experience highly preferable

Responsibilities

  • Admit patients to hospice services.
  • Coordinate with the patient's community/nursing home physician and hospice medical directors to confirm eligibility for hospice services.
  • Complete the initial assessment of patient/family needs at nursing homes.
  • Order medications, medical equipment, supplies, and support from the interdisciplinary team.
  • Ensure a smooth hand-off to the Hospice Case Manager.
