About The Position
As a Community Health Nurse, you will provide nursing interventions, patient education, and care management to a variety of clients in the community. The role promotes good health and provides one-on-one patient care in clients’ homes. The position is in Home Care at Rochester Regional Health Home Care, 330 Monroe Avenue, Rochester, NY 14607. It is per diem, with a days schedule of 8:00 a.m. to 5:00 p.m. as needed.
Requirements
- Diploma or Associate’s Degree in Nursing.
- Registered Nurse license in New York State.
- Valid New York State Driver’s License.
Nice To Haves
- Bachelor’s Degree in Nursing.
- Two years of experience in home care.
- Bilingual ability.
- At least one year of nursing experience.
Responsibilities
- Provide comprehensive assessments of the bio-psycho-social needs of acutely ill clients in their homes.
- Perform skilled nursing care and prescribed treatments based on physician orders.
- Provide client and family education in accordance with the client assessment and plan of care.
- Develop and document individualized care plans tailored to each patient, with interdisciplinary health team support as needed.
- Communicate patient health status, treatment plans, and progress effectively.
- Communicate regularly with office and supervisory staff.
- Coordinate client services and referrals efficiently and promptly.
- Accurately and completely document client care.
- Document and implement physician orders in accordance with Department of Health and program standards.
- Follow department and system protocols, applicable local, state, and federal regulations, and education requirements.
