This role provides leadership to the health care team with an emphasis on coordination of patient care and throughput of patients. The Clinical Lead demonstrates technical and clinical competence, serving as a clinical resource and role model for staff. They facilitate the development of safe and effective plans through early identification of patient needs and resources, anticipating medical and psychosocial needs in high-risk patients. The position involves facilitating appropriate clinical pathways and plans of care, participating in daily rounds with a focus on discharge planning, and serving as charge nurse and operational leader for patient flow when not in an administrative role. The Clinical Lead ensures patients and caregivers are aware of the patient care plan and anticipated discharge date, responds to customer concerns, and alerts physicians of cases needing review. They act as a liaison between patient, family, physician, and the interdisciplinary health care team, and work with the Clinical Intake and Navigation Center for efficient patient flow. This role also involves reviewing and amending staffing to meet immediate and future needs, facilitating safe discharge plans, and engaging with patients, families, and post-acute care for follow-up actions. Additionally, the Clinical Lead participates in staff performance evaluations, aligns educational activities with organizational goals, assures timely communication and provision of educational activities, and works with unit leadership to facilitate orientation for new staff, ensuring checklists are completed and stored appropriately.
