About The Position

As a RN Clinical Field Staff Liaison (CFSL), the work you do every day makes a difference in the lives of our patients by ensuring quality and safe delivery of home health services. You’ll provide support and act as a resource to clinicians in the field. You will assist in monitoring compliance quality and safe delivery of services by following a clinical process set by clinical management.

Requirements

Minimum one-year home health experience is preferred
Current RN license in the state of employment
Minimum of Three (3) years of experience as an RN in an acute care setting
Participates in on-call, evening/weekend shifts and provides patient care, per agency needs
Valid driver's license and auto insurance in your name as a driver
Capable of all physical demands

Responsibilities

Serve as a resource for field staff, referral sources, physicians, patients and their families pertaining to clinical issues and/or concerns.
Facilitate calls to and from Field staff; uses resources to resolve issues and reports anything of concern to manager.
Monitor Home Health Aide supervisory visits.
Work in the field with staff and patients as needed.
Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families.
Ensure Start of Care compliance and quality and safe delivery of home health services.
Focus on Outcome Monitors as assigned.
Implement quality improvement programs in the field.
Review clinical documentation and maintain current knowledge of all Medicare/ Federal/ Insurance / Company and Hospital guidelines, policies and protocol.
Assist with monthly Case Manager, Nurse and Therapy meetings.
Participate in all on-call requirements, and patient staffing as needed.
