The mission of this role is to deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Responsibilities

  • Deliver excellent home health care to patients in Detroit.
  • Set the clinical bar, documentation standard, and referral-source trust.
  • Carry a full, high-quality caseload, ramping up to ~30 productivity points/week at target visit quality, with OASIS accuracy.
  • Own patient outcomes, driving low avoidable-hospitalization rates and strong patient-experience scores.
  • Participate in case conferences/team huddles to improve quality.
  • Act as a referral-source ambassador, ensuring responsiveness and excellence.
  • Set the standard for charting, cadence, and judgment for new team members.
  • Perform comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
  • Provide direct, high-acuity patient care, with a focus on wound care.
  • Build and manage the plan of care with physicians.
  • Supervise LPNs and HHAs on assigned cases.
  • Serve as the face of Adaptive to patients, families, and referral sources.
