Responsibilities

Deliver excellent home health care to patients in Detroit.
Set the clinical bar, documentation standard, and referral-source trust.
Carry a full, high-quality caseload, ramping up to ~30 productivity points/week at target visit quality, with OASIS accuracy.
Own patient outcomes, driving low avoidable-hospitalization rates and strong patient-experience scores.
Participate in case conferences/team huddles to improve quality.
Act as a referral-source ambassador, ensuring responsiveness and excellence.
Set the standard for charting, cadence, and judgment for new team members.
Perform comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
Provide direct, high-acuity patient care, with a focus on wound care.
Build and manage the plan of care with physicians.
Supervise LPNs and HHAs on assigned cases.
Serve as the face of Adaptive to patients, families, and referral sources.
