About The Position

Superior Home Care and Hospice is a locally owned and operated hospice and home health agency with 27 years of experience and an excellent reputation for providing high-quality, patient-focused care. They are seeking a qualified Hospice and Home Health RN Case Manager who shares their values and wants to contribute to their vision of providing excellent community care. The company prides itself on low employee turnover and is looking for a qualified Part-Time RN Case Manager for a long-term position. This role offers flexibility with 20-25 hours per week and is compensated at a per visit rate. The position is described as both challenging and rewarding, requiring a team-oriented individual.

Requirements

Licensed as a Registered Nurse by the State of Utah
Current Utah driver's license and automobile insurance
CPR certification
A minimum of 1 year nursing experience is required
Good documentation skills
Passion for providing high-quality patient care
Enjoy caring for patients in a home setting

Nice To Haves

Hospice and home health experience is preferred but not required

Responsibilities

Provide skilled nursing care including wound care, catheter care, venipuncture, observation and assessment, medication compliance and teaching, hospice assessments and other skilled care.
Create patient-centered goals with patients, family members, and providers.
Assess, supervise, and direct patient care.
Develop plans of treatment.
Ensure chart compliance with state regulations.
Coordinate patient care with physicians and other providers.
