The Registered Nurse (RN) provides comprehensive skilled care as ordered by the attending physician. The RN educates patients and their family members and ensures the safety of the patient. The RN is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The RN is responsible for all practices and duties within the scope of practice as outlined by the state. This role involves providing skilled nursing care in a patient’s home, performing OASIS assessments, administering medication, managing multi-disciplinary care, and promoting continuity of care. The RN will counsel patients and families, incorporate them into the plan of care, teach preventative measures, listen to concerns, and supervise the care provided by Aides and LPNs. The RN evaluates treatments and the patient's medical condition, determines if the level of care meets patient needs, communicates supervisory visit results, and completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines. The RN must maintain an active RN license, communicate changes to the scheduler, participate in on-call duties and case conferences, attend trainings and meetings, and maintain a professional appearance. Essential functions include driving to patient locations, daily attendance at assigned visits, documenting visits within specified timeframes, following the plan of care, and fulfilling RN visits in the assigned geographic location.
