About The Position

The Registered Nurse (RN) Case Manager monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance. This role involves consulting regularly with nursing staff and the multidisciplinary team to evaluate patient status and appropriateness of care, including admission, length of stay, transfers, and discharge. The RN Case Manager also monitors patient and family satisfaction, responds to inquiries, leads and coordinates discharge planning, performs chart reviews, and communicates effectively with providers, payors, and ancillary services. Accurate documentation of case management activities, plans, and outcomes is essential, as is participation in performance improvement and continuing education. The role may require working alternate shifts/units and may include on-call responsibilities.

Requirements

  • Current Tennessee or compact state RN license.
  • Associate’s degree from an accredited nursing school.
  • AHA Basic Life Support (BLS) within 30 days of hire.
  • Critical thinking skills.
  • Decisive judgment.
  • Ability to work with minimal supervision.
  • Ability to work in a stressful environment and take appropriate action.
  • Licensed Registered Nurse with the State of Tennessee or a licensed Registered Nurse from a Compact State with the proper Tennessee paperwork filed within 30 days of hire.
  • Associate's degree required.

Nice To Haves

  • BSN preferred.
  • One-year previous experience preferred.

Responsibilities

  • Monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.
  • Consults regularly with nursing staff and the multidisciplinary team to evaluate patient status and appropriateness of care, including admission, length of stay, transfers, and discharge.
  • Monitors patient and family satisfaction; responds to questions and complaints from patients, families, and payors.
  • Leads and coordinates discharge planning, including patient transfers to other facilities and referrals to community resources; provides discharge education.
  • Performs chart reviews to identify and address issues related to service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.
  • Communicates effectively with providers, payors, and ancillary services to remove barriers and ensure timely care progression.
  • Accurately documents case management activities, plans, and outcomes.
  • Participates in performance improvement and required continuing education.
  • Supports department operations by working alternate shifts/units as needed; may be assigned on-call.
  • Performs other duties as assigned.
