About The Position
We’re hiring a Registered Nurse (RN) Case Manager who wants a predictable weekday schedule, meaningful work with older adults, and the flexibility of a hybrid position. You’ll step into a community-based care management role focused on developing individualized care plans, coordinating services, and helping older adults remain safely in their homes. This is a care management position rather than a bedside nursing role, giving you meaningful autonomy with interdisciplinary support.
Requirements
- Clear and active Massachusetts RN license
- At least 2 years of RN experience
- Current CPR certification
- Comfortable working independently in a community-based setting
- Enjoy care coordination and working with older adults
- Value collaborative, patient-centered care
