About The Position
Home Health Care, Inc. is seeking a dedicated Registered Nurse (RN) Case Manager to manage a home health caseload within a dedicated primary territory in the western Twin Cities. This role is designed to minimize travel time, allowing nurses to focus more on patient care and building relationships within the community. The position offers independence in the field with strong clinical support, manageable caseloads, and a collaborative team environment. Home Health Care, Inc. is a long-standing home health organization with over 30 years of service, known for its supportive culture and understanding leadership.
Requirements
- Active Minnesota Registered Nurse (RN) license.
- 3+ years of RN experience.
- Strong assessment, communication, and organizational skills.
- Ability to work independently while managing a field-based schedule.
- Reliable transportation.

- Home health experience preferred.
Responsibilities
- Manage a home health caseload throughout your assigned territory.
- Complete OASIS assessments and individualized care plans.
- Deliver skilled nursing care and patient education.
- Collaborate with physicians and interdisciplinary team members.
- Help patients remain safe, healthy, and independent at home.
- Maintain timely and accurate clinical documentation.
