Registered Nurse (RN) Case Manager, Hospice
About The Position
Phoenix Home Care & Hospice is seeking a hospice RN Case Manager to provide compassionate nursing care to clients and families facing a terminal diagnosis. The nurse works with the interdisciplinary team to develop and implement patient- and family-centered plans of care, discuss goals of care, and guide healthcare decisions to support quality of life. The role is full-time, Monday through Friday, 8:00 a.m. to 5:00 p.m., serving Jefferson City, Missouri, and surrounding areas. The listed pay range is $65,000–$90,000. Requirements
- Missouri or Compact RN license.
- Hospice or long-term nursing experience.
- Valid driver's license and auto liability insurance.
Responsibilities
- Provide patients and family members with education about the process and plan of care, and guidance with healthcare decisions.
- Consult with the attending physician and Hospice Medical Director about changes in a patient's condition that require order or plan-of-care changes.
- Coordinate hospice care with team members and patients and family members to ensure needs are met.
- Conduct initial and ongoing comprehensive assessments.
- Evaluate patients for hospice care and conduct informational visits.
