About The Position

At Saint Francis Hospital, our RN Case Managers are responsible for coordinating patient care, assessing needs, and developing care plans. They collaborate with healthcare providers, monitor patient progress, and ensure a smooth transition between different stages of care while advocating for the patients' well-being. As a Registered Nurse RN Case Manager, you will serve as a driver of a collaborative process which coordinates, monitors, and plans health care services throughout the patients’ illness.

Requirements

• Bachelors of Science in Nursing (BSN) degree
• Current Licensure as a registered nurse (RN) in Connecticut
• Five years of diversified, progressive experience in acute care and/or other settings within the continuum of Care
• Understanding of nationally recognized medical utilization criteria
• Understanding of HCFA/OBRA and state of Connecticut regulatory compliance issues
• Understanding of JCAHO accreditation accountabilities
• BLS certification
