About The Position

This role involves providing skilled nursing care in a patient's home as directed by the attending physician. The Registered Nurse (RN) Case Manager will work autonomously, requiring efficiency in time management and adherence to the scope of practice outlined by the state. The position is part of The Christ Hospital Home Health Care and the Alternate Solutions Health Network family.

Requirements

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.
