Registered Nurse (RN) Case Manager

Requirements
- Current Registered Nurse (RN) license in the State of New Hampshire
- Current CPR Certification
- Valid driver's license and reliable transportation
- Strong clinical assessment, communication, organizational, and critical-thinking skills
- Ability to work independently while collaborating effectively with a multidisciplinary team
- Compassionate, patient-centered approach
- Genuine desire to make a difference in the lives of patients and families
Nice To Haves
- Minimum of two (2) years of skilled nursing experience preferred
- Home health experience preferred, but we are willing to consider strong candidates with relevant clinical experience
- Previous nursing supervisory experience preferred
- Recent wound care experience preferred
Responsibilities
- Perform comprehensive nursing assessments and develop individualized plans of care.
- Provide skilled nursing visits and ongoing case management.
- Monitor patient progress and identify changes in condition.
- Coordinate care with physicians, therapists, caregivers, and other healthcare professionals.
- Educate patients and families on disease management, medications, treatments, and strategies for maintaining independence.
- Provide compassionate support to patients and families during important moments in their healthcare journey.
- Complete accurate and timely clinical documentation.
- Supervise and collaborate with members of the interdisciplinary care team.
- Advocate for patients and ensure continuity, quality, and safety of care.
- Help patients achieve their goals and remain safely in the place they call home.
