About The Position

This role is for a Registered Nurse (RN) Case Manager/Swing Bed Coordinator at Highpoint Health Sumner. The position is full-time and involves coordinating and marketing Swing Bed admissions for Trousdale Medical Center and Riverview. The RN will monitor the care of patients within case groups to ensure high quality and cost-effective care, and work with other hospital personnel to ensure appropriate and timely discharge planning. The position reports to the Director of Case Management and is classified as Exempt.

Requirements

• Graduate from an accredited school of nursing
• Current RN license to practice/work in the state of TN
• Basic Life Support (BLS) - to be obtained within 30 days
• Two years recent related clinical experience preferred in Utilization Review, Quality Improvement and Case Management
• Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
• Must be able to work in a stressful environment and take appropriate action.

Responsibilities

• Coordinating/marketing Swing bed admissions for Trousdale Medical Center and Riverview.
• Monitoring care of patients within case groups to ensure high quality and cost-effective care is provided.
• Working with other hospital personnel to assure appropriate and timely discharge planning.
• Consulting with nursing staff and multidisciplinary team regularly to evaluate patient's status and appropriateness of medical care, including admission, length of stay, transfer and discharge.
• Monitoring patient and family satisfaction. Responding to questions and complaints from patients, family members, and payors regarding care.
• Participating in discharge planning including coordinating patient transfers to other facilities and coordinating community resources.
• Providing discharge education and resource referrals to patients.
• Performing chart review to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.
