About The Position

Right at Home Boston Northwest is seeking a compassionate and organized Registered Nurse (RN) Case Manager to join their home care team. This is a field-based position serving clients in the Boston Northwest service area, requiring local travel between client homes. The RN Case Manager will work with the Director of Nursing and care team to oversee the care of older adults who wish to remain safe and independent in their homes. This role is ideal for an RN who enjoys clinical assessment, care coordination, direct family and caregiver interaction, and greater independence than a traditional bedside role.

Requirements

• Active, unrestricted Registered Nurse (RN) license in Massachusetts
• 2+ years of nursing experience
• Strong clinical assessment and judgment skills
• Excellent communication and interpersonal skills
• Strong organization, time-management, and documentation skills
• Comfortable working independently while remaining part of a collaborative clinical team
• Compassionate and dependable, with an interest in working with older adults and their families
• Valid driver's license and insured vehicle
• Ability and willingness to travel locally between clients' homes

• Home care, geriatrics, private-duty nursing, or case-management experience

Responsibilities

• Conduct in-home nursing assessments and reassessments for new and existing clients
• Develop, review, and update individualized plans of care
• Provide ongoing clinical case management and follow-up visits
• Monitor changes in client condition and coordinate appropriate clinical follow-up
• Educate, coach, and support caregivers in delivering safe, high-quality care
• Serve as a clinical resource for clients, families, caregivers, and the office team
• Collaborate with the Director of Nursing, scheduling, and care management teams to maintain continuity of care
• Communicate with physicians and other healthcare professionals when appropriate
• Support clients and families during changes in care needs and transitions
• Maintain accurate and timely clinical documentation
• Build positive relationships with clients, families, caregivers, and community healthcare partners
