About The Position

Mercy Health St. Vincent Medical Center is a faith-based Catholic Healthcare Ministry committed to serving the poor and underserved of our community since 1853. It is a Regional Tertiary care facility serving a 150-mile radius of Northwest Ohio and Southeast Michigan. St. Vincent has been a well-established leader in quality healthcare as demonstrated by being verified as a Level I Adult and Level II Pediatric Trauma Center by the American College of Surgeons Committee on Trauma, designated as a STEMI Center, as well as Comprehensive Stroke Center verified by the Joint Commission. Mercy Health St. Vincent Medical Center has also partnered with Nationwide Children’s Hospital of Columbus to create Nationwide Children’s Hospital of Toledo on the campus of Mercy Health St. Vincent Medical Center. The Registered Nurse (RN) in Case Management manages patient transitions of care through the hospital and then to the appropriate post-acute setting. This role assures third-party payer approval for any services provided and ensures appropriate documentation to support patient status, level of care, and DRG.

Requirements

  • Current RN license in the state of Ohio.
  • Must have a minimum of 3 years of clinical experience.
  • Current CPR certification required.
  • ACLS and other certifications may be required.

Nice To Haves

  • BSN preferred, or must be obtained within 3 years.
  • Clinical specialty or case management certification preferred.
  • Case Management experience preferred.
  • Case Management certification preferred and required within 3 years of hire date.
