This is a full-time Registered Nurse (RN) Care Manager position within the Community Care Partners - Administration Department. The role is primarily remote, with standard Monday-Friday day hours. While not strictly required, a preference is given to nurses holding WI and MI licenses. The position involves facilitating communication among patients, families, multidisciplinary teams, and community resources to address and resolve barriers to care delivery. The RN Care Manager will interpret contracts to optimize clinical and financial outcomes, and collaborate with various care teams to ensure quality, efficient, and cost-effective healthcare services. The role utilizes evidence-based approaches to enhance patient and family engagement in their own care, developing SMART goals and identifying potential learning or care delivery barriers. The nurse will report abnormal findings to providers, manage their caseload independently, and ensure timely completion of documentation for regulatory and contractual requirements. This role partners with at-risk patients throughout their care journey to ensure continuity and anticipates patient needs, responding to changes in status and prioritizing care. Coordination of patient information and communication between various entities, including referring facilities, physicians, and community caregivers, is essential for smooth transitions of care. Referrals to internal and external resources will also be coordinated. The position requires the ability to provide age-appropriate care, demonstrating knowledge of growth and development across the lifespan and assessing patient status based on age-specific needs.
