The RN Care Coordinator will serve as a core clinical member of The OSU Complex Care Program, working with patients who have childhood-onset, medically complex conditions. These conditions commonly include technology dependence (e.g., tracheostomy, G/GJ tubes), enteral nutrition needs, and multisystem chronic disease. This role emphasizes high-level clinical triage, relationship-centered care, interdisciplinary coordination, and involvement in program development, including research, quality improvement, and innovative care processes. Requirements

  • Associate Degree in Nursing from a CCNE (Commission on Collegiate Nursing Education) or Accreditation Commission for Education in Nursing (ACEN) accredited nursing program .
  • Current Ohio Registered Nurse license.
  • CPR/BLS certified.
  • Knowledge of triage procedures along with clinic, physician office and/or urgent care experience.
  • Ability to maintain quality, safety, and/or infection control standards.
  • Knowledge of appropriate procedures and standards for the administration of medications and patient care aids.
  • Ability to educate and clearly communicate with patients and/or families as to the nature of disease and to provide instruction on proper care and treatment.
  • Ability to convey information both in writing and verbally.
  • High level of interpersonal skills to handle sensitive and confidential situations.
  • Position continually requires demonstrated poise, tact and diplomacy.
  • Analytical ability and critical thinking to gather and interpret data and develop, recommend, and implement solutions

Responsibilities

  • Provide telephone triage for pediatric and adult patients (ages 0–100) with medically complex needs.
  • Assess symptoms, determine acuity, and coordinate care plans in collaboration with attending physicians and multidisciplinary specialists.
  • Facilitate care transitions, follow-up planning, and communication among interdepartmental and interinstitutional teams.
  • Provide hands-on clinical support during emergency situations.
  • Perform (as needed during emergencies): Urinary catheterization Dressing changes and basic wound care Ear irrigation Support during emergency events requiring BLS, including seizures or respiratory compromise Use of patient lift devices and wheelchair-accessible scales
  • Participate in quality improvement (QI) initiatives, data tracking, and development of new workflows, clinical pathways, and supportive documentation tools (templates, flowsheets).
  • Engage in research-supporting activities, including process evaluation and optimizing patient experience.
  • Contribute to strategy development for a growing, innovative, patient-centered complex care model.
  • May need to travel to other departments within OSU, or at our pediatric collaborating institution as needed for learning and education the team on interinstitutional platform, scheduling and triaging frameworks.
  • Provide skilled nursing services to patients under the direction of a physician or midlevel provider, and provides a variety of related services to maintain a safe, therapeutic environment.
  • Collect patient health data, analyze the data to determine diagnoses, identify expected outcomes individualized to the patient,
  • Develop a plan of care that prescribes interventions to attain the expected outcomes,
  • Implement the interventions identified in the plan of care, and evaluate the patient’s progress toward attainment of the outcomes,
  • Work collaboratively with the healthcare team.
  • Perform professional nursing work of considerable difficulty including the care of patients as outlined by physicians and the objectives and policies of the organization.
  • Provide patient and family health education and health assessment.
