Make a Difference As a member of the team, you will have the opportunity to make a profound impact on the lives of people living with complex and/ or chronic conditions. This position is primarily remote; however, you may connect with your patients in the physician office setting, on the phone, at their home, or in the hospital setting - essentially however and wherever the patient needs your assistance to improve their health, better understand their illness and coordinate their care.

Requirements

  • Graduate of Accreditation Commission for Education in Nursing or Commission on Collegiate Nursing Education accredited school of nursing.
  • Licensed as a Registered Nurse by the Indiana Professional Licensing Agency required.
  • 3+ years of nursing experience required.

Nice To Haves

  • Preferably in Population Health management; home health, ambulatory care, community public health, case management, and coordinating care across multiple settings and with multiple providers.
  • Experience with electronic health records, EPIC experience preferred.
  • Knowledge and experience teaching self-management preferred.
  • Experience with end-of-life care, palliative care, hospice highly preferred.
