About The Position

This role serves as a Cardiovascular Outpatient RN in multiple capacities within the SMC OP Heart & Vascular Center. The Registered Nurse will be responsible for patient care in Cardiac and Pulmonary Rehab, the Heart Failure Clinic, and the Outpatient Heart and Vascular Center. Responsibilities include assessing, developing, and implementing care plans, patient education on risk factor reduction, activity progression, and dietary modifications. For the Heart Failure Clinic, the RN will provide direct age-specific and culturally appropriate patient care, serve as a clinical resource, and participate in patient and staff education. In the Outpatient Heart and Vascular Center, the RN will assist in minimally invasive interventional and diagnostic cardiovascular procedures, including diagnostic imaging, atherectomy, peripheral angioplasty, CTO treatment, intravascular imaging, vein ablations, stenting, TEEs, cardioversions, and implantation/removal of subcutaneous rhythm monitors. Extensive training will be provided due to the specialized nature of the role. The role requires a team-oriented individual with strong communication skills.

Requirements

  • Graduate of an accredited nursing program
  • Associates Degree in Nursing
  • Minimum of 2 years cardiac experience
  • Current license to practice Nursing in the State of South Carolina
  • Certification in Basic Life Support
  • Advanced Cardiac Life Support (for Cardiac Rehab and Outpatient Heart and Vascular Center only)

Responsibilities

  • Assesses, develops, and implements plan of care and measures patient response to exercise prescription.
  • Provides patient education concerning cardiac and pulmonary risk factor reduction, activity progression, and dietary modification for adults.
  • Utilizes knowledge and skills to provide direct age-specific and culturally appropriate patient care for congestive heart failure patients.
  • Serves as a clinical resource and participates in patient and staff education.
  • Demonstrates leadership and serves as a preceptor.
  • Assists in performance of minimally invasive interventional and diagnostic cardiovascular procedures.
  • Provides pre/post procedure care as well as intra-procedure care of patients.
  • Collaborates with patients, families, and other members of the health care team to determine patient priorities and treatments.
  • Initiates and facilitates interdisciplinary collaboration to enhance outcomes.
  • Seeks necessary assistance in completing nursing interventions to assure positive patient outcomes.
  • Responsible for demonstrating competency for all new equipment and procedures.
  • Provides clinical guidance to students and ancillary staff.
  • Successfully completes hospital and department-based orientation programs.
  • Formulates a plan for professional growth.

  • Communicates effectively with all members of the care delivery team.
  • Responsible for participating in process improvement projects within the department.

  • Adheres to and practices appropriate infection control policies and procedures.
  • Performs accurate, legal, and ethical documentation at all times.
  • Successful completion of an Arrhythmia course.
  • Completion of critical care course desired.
  • Orientation for new patients including creating initial exercise prescription and developing Individual Treatment Plans.
  • Assists with inpatient consultations and education and assisting with referrals as assigned.
  • Responsible for review and maintenance of referral work queue.
  • Triages patient calls and electronic messaging from patients regarding signs and symptoms of heart failure.
  • Triages patient calls regarding medication refills and medication questions.
  • Assists in prior authorization process for medications and non-invasive procedures (echo, stress test).
  • Completes and follow up on patient financial assistance applications.
  • Measures and records vital signs, takes patient medical history and chief complaint. Records information accurately in the medical chart.
  • Prepares exam and treatment rooms with necessary supplies.
  • Prepares and maintains supplies and equipment for treatments, including ordering supplies.
  • Assists with scheduling of tests and treatments.
  • Screens, manages, and follows up telephone messages from patients, referrals, and pharmacies.
  • Prepares and sends prescriptions electronically per provider's orders.
  • Performs lab draws, EKGs per provider's orders.
  • Explains patient's problems, medications, and procedures to patient.
  • Provides patient education regarding heart failure during patient appointments.
  • Teaches weekly in-person education class to patient and caregivers regarding heart failure.
  • Uses printed handouts, booklets, samples, examples, and videos computer programs as applicable.
  • Helps to maintain patient education material. Reviews educational material as needed.
  • Assists with inpatient rounding, referrals, education as needed.
  • Provides Patient Access Representative (PAR) coverage when needed.
  • Disposes of biohazard waste according to state standards.
  • Successful completion of hospital approved arrhythmia course.
  • Removes venous and arterial sheaths and provides post-sheath removal care.
  • Provides pre and post procedure education to patients and families.
  • Prepares treatment rooms and lab as required.
  • Measures and records vital signs and hemodynamics.
