About The Position

This role involves performing an age-specific plan of care for a designated group of patients using the nursing process. The Registered Nurse will collaborate with physicians and other health team members to coordinate and implement procedures and treatments. Leadership skills and clinical judgment will be utilized in coordinating patient care and directing the activities of the patient care unit team.

Requirements

  • 2+ Years of Antepartum RN experience required.
  • BLS, ACLS, NRP Advanced & AWHONN Intermediate Required.
  • Graduate from an accredited School of Nursing.
  • Associate’s Degree in Nursing, required.
  • Current license to practice as a Registered Professional Nurse in New York State and/or Connecticut, required.
  • BLS required.

Nice To Haves

  • Bachelor’s Degree in Nursing, preferred.
  • NCC-EFM Certification must be obtained within 1 year of hire.

Responsibilities

  • Assesses the patient’s physiologic health status, including conducting patient interviews, explaining policies and procedures, reviewing charts, assessing various health statuses (gastrointestinal, cardiovascular, respiratory, renal, neurological), determining mobility, sensory deficits, prostheses use, and skin condition, assessing level of pain and pain management, and communicating/documenting the patient’s physiologic health status and plan of care.
  • Assesses the patient’s psychosocial health status, including eliciting perception of medical/nursing care, determining coping mechanisms, knowledge level, and ability to comprehend, identifying cultural/ethnic requirements, and communicating and documenting psychological status and care plan.
  • Identifies patient outcomes by developing criteria for measurement, identifying actual/potential patient problems, identifying patient’s need for teaching, and developing patient outcome statements and individualized patient goals.
  • Formulates the plan of care and establishes priorities necessary to achieve expected outcomes, including identifying care activities, coordinating cost-effective use of supplies, equipment, and medication, and documenting the plan of care in collaboration with physicians and other health team members.
  • Implements the plan of care by maintaining vigilance over patient safety, exercising professional skills, reassessing patients as needed, correctly administering prescribed treatments and medications, correctly using equipment, providing emotional support, applying scientific principles, making accurate observations, keeping accurate documentation, acting rapidly and effectively during emergencies, providing a calm atmosphere, communicating effectively with family/caretakers, participating in discharge planning, and providing discharge instructions.
  • Evaluates care provided for patient outcome by demonstrating ability to measure effectiveness of care, performing variance analysis, designing/implementing/evaluating systems to improve care, keeping accurate documentation, and using appropriate documentation methods.
  • Collaborates with other care team members in planning and carrying out treatment regimens, providing direction to other members of the care team, collaborating with physicians, accurately interpreting and implementing treatment regimens, assisting physicians, keeping the Patient Care Manager/designee and/or physician abreast of changes in patient’s condition, and using clinical judgment in delegating assignments.
  • Performs grade I-IV Decubitus Care, including performing preventive skin care measures, applying simple dressings, applying warm & cold compresses, performing irrigations, and performing other procedures related to skin care.
  • Administers medications correctly and safely, including correctly identifying medication, action, dosage, side effects, and implications for the patient, meeting standards on medication administration examination, demonstrating preparation of local solutions and I.V. drips, administering and documenting medication correctly, administering IVP medication as per policy, and educating patients and significant others related to drug and food interactions.
  • Participates in patient and family education by providing explanations of the patient’s condition, communicating assessment data, recognizing and utilizing health teaching opportunities and resources, providing for early discharge planning and appropriate referrals, and evaluating the effectiveness of teaching.
