About The Position

The After-Hours RN provides clinical support outside of traditional business hours and helps ensure continuity of care while reducing unnecessary hospitalizations. This role offers a flexible schedule with evening-focused work, greater work-life balance, and an independent patient care environment, avoiding hospital shifts or 12-hour rotations. The position utilizes technology such as AI-assisted charting and a user-friendly EMR (WellSky/Kinnser) to reduce documentation time and allow more patient interaction. Requirements

Active Florida RN license or Compact RN license
Strong assessment and triage skills
Ability to work independently
Reliable transportation, valid driver's license, and auto insurance

Responsibilities

Complete skilled nursing visits during weekday evenings
Perform Start of Care (SOC) assessments
Perform Resumption of Care visits
Perform PRN visits
Perform Urgent nursing visits beginning at 3-4:00 PM
Respond to after-hours patient calls and triage concerns appropriately
Coordinate care with Physicians, Case managers, and Interdisciplinary care teams
Provide skilled clinical care including comprehensive patient assessments, medication reconciliation, wound care, disease management education, and catheter care
Support urology-related patient needs including Foley catheter management, nephrostomy care, urinary retention monitoring, post-surgical urology follow-up, and infection prevention education
Complete documentation within required EMR timeframes
