Banner - University Medicine Advanced Heart Failure Clinic provides state of the art comprehensive, evidence-based care with the latest technologies including Mechanical Circulatory Support, Artificial Heart and Heart Transplantation. Our multidisciplinary team is focused on providing compassionate care, resources and expertise tailored to meet each patients’ unique needs. As part of an academic medical center, we offer access to some of the newest treatments being tested in clinical trials. This is an innovative, autonomous role, with continuous learning opportunities in a great team environment providing heart failure patients and caregiver education with goals of improving outcomes, decrease length of stay, readmission and ED utilization. Inpatient: multidisciplinary team daily rounds, weekly participation in advanced therapy selection committee, discharge planning and education, addressing/removing and preventing barriers for patients who have barriers to advanced therapies, collaboration with other disciplines (addiction medicine, palliative, family medicine, bariatrics, endocrinology). Support and education for bedside nurses related to heart failure needs. Clinic: follow-up hospital discharge appointments, reinforcing education, assess for decompensation to identify opportunities for early intervention, management of inotrope patient visits (lab/IV starts/IV diuretic admin/education), pulmonary artery pressure monitoring device screening/education/tracking, triage phone calls. This is a full time (40 hours/week), day shift position: Monday - Friday day shift, 7am-4pm. University Medical Center Phoenix is a nationally recognized academic medical center. POSITION SUMMARY This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. This position utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.
