About The Position

Delivers direct/indirect patient care to adult acute inpatient rehabilitation patients utilizing the nursing process through data collection and assessment; implementation and prioritization of patient’s problems and needs; and development/implementation/evaluation of a nursing plan of care. Focuses on the unique needs of the rehab patient while transitioning them from an acute/post-acute care setting to a community setting. Rehabilitation nurses are part of an interdisciplinary team and coordinate care with therapists, case managers, social workers, and a Physiatrist (Physical Medicine and Rehabilitation Physician) to assess the unique rehab needs that address deficits, implement a rehab nursing plan of care with comprehensive interventions and develop ongoing caregiver and family education. They must demonstrate critical thinking to assure the delivery of safe, quality, age-specific care while delegating effectively to appropriate team members. Rehab nurses are expected to document accurately and in a timely manner while utilizing the CARE tool as mandated by the Centers for Medicare/Medicaid Services. The rehab nurse is expected to adhere to organizational policies and procedures; regulatory accrediting body requirements; and professional practice standards.

Requirements

  • Current knowledge of the nursing role and scope of practice as defined by the State of Texas Nurse Practice Act and the Texas Board of Nursing.
  • Current knowledge of American Nurses Association (ANA) Code of Ethics.
  • Knowledge of current trends and demonstration of standards of care in assigned unit.
  • Successful completion of mandatory requirements including unit proficiencies/competencies.
  • Demonstration of effective problem solving, decision-making, interpersonal, and teamwork skills.
  • Demonstration of effective verbal and written communication.
  • Demonstration of positive customer service.
  • Graduate of an accredited program for nursing education, BSN preferred.
  • Current licensure to practice professional nursing in the State of Texas.
  • Current American Heart Association (AHA) Healthcare Provider CPR (BLS) certification required.

Nice To Haves

  • Certification in specialty is preferred; other as noted below for specific units as indicated within 2 years of hire: Certified Rehabilitation Registered Nurse (CRRN) through the American Nurses Credentialing Center and the Association of Rehab Nurses.
  • Previous work experience in a patient setting preferred.
  • ARU-1 year of medical-surgical and 1 year home health experience preferred

Responsibilities

  • Assesses/reassesses patient’s condition based upon observation of patient and interview of patient and/or family, physician’s diagnosis and data, diagnostic results, and relevant data from other healthcare providers. Assessment and reassessment of functional and cognitive deficits as it relates to transition to the community.
  • Uses professional knowledge to develop/revise nursing plans of care for assigned patients according to unit guidelines and criteria with expected outcomes.
  • Works with rehab team members to construct, develop, and implement rehabilitation treatment plan and rehabilitation plan of care to address emergent medical issues as well as functional/self-care/cognition deficits.
  • Delivers safe rehabilitation nursing interventions in an organized, efficient, and prioritized manner to address identified needs.
  • Attends weekly team conference to report on assigned patients’ status and progress toward rehab goals and medical status.
  • Evaluates/re-evaluates patient’s response to rehab nursing interventions and patient’s progress to expected outcome(s) and community discharges.
  • Collaborates and communicates effectively with team members, physicians, and other healthcare providers to coordinate the medical and nursing plans of care.
  • Accepts delegated responsibilities for functions such as, but not limited to, charge nurse for unit activities on assigned shift.
  • Delegates responsibilities to appropriate personnel.
  • Documents all aspects of patient care, maintains patient records and care plans in a timely manner as appropriate for unit including scoring the CARE tool and providing data for the quality indicators as mandated by CMS.
  • Accurately complete Section GG scoring at admission, discharge, and per IRF-PAI guidelines.
  • Provides patients and family members with relevant health care education and instructions regarding treatments and procedures including pre/post hospital care as appropriate.
  • Assist in the orientation and training of new and tenured personnel, reassigned nurses, student nurses, Patient Care Assistants, and faculty; serves as a preceptor when requested.
  • Maintains self-control under stressful situations and workload pressures without exhibiting negative behaviors.
  • Demonstrates knowledge of disaster/emergency procedures and responds appropriately.
  • Assumes responsibility and accountability for individual knowledge, skills, performance and behavior in accordance with hospital, division, and unit standards of care and policies and procedures.
  • Utilizes measures to promote and maintain patient, visitor and personnel safety.
  • Maintains patient, employee, physician, and organization confidentiality; respects the rights, privacy, and property of others.
  • Demonstrates the appropriate use of infection control principles and personal protection equipment.

  • Operates as a team member planning, coordinating, developing, and implementing a rehab team care plan.
