The Referral Coordination Team (RCT) Program Manager Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to program management. The RCT Program Manager performance and leadership are broad enough to impact the standard of care and the safety of staff and patients across the continuum of care. They are accountable for planning, organizing, directing, and evaluating the program and the personnel who support it. In supervising the RCT Nurses, the RCT Program Manager leads the team in executing a streamlined approach to receiving, triaging, and directing timely, coordinated care to assess, organize, facilitate, and guide Veterans through their full range of care options including internal/direct care in Veterans Affairs (VA) and care in the community as a liaison to both internal and community providers and is responsible for managing the care of the Veteran throughout the consult process. Key responsibilities include managing team performance, conduction reviews, ensuring compliance with regulations and guidelines, collaborating with providers and internal staff, analyzing data for process improvement. Serves as the facility subject matter expert for relevant medical policies and RCT processes.
