About The Position

One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services. We are currently seeking a compassionate, committed 1FTE Registered Nurse to work with the Primary Care team as well as in the Rapid Access Addiction Medicine (RAAM) Clinic. This position will provide nursing services within NorWest CHC’s Primary Care and the RAAM team. Within the RAAM clinic, the RN provides low-barrier addictions care to clients and their families. The approach to care is strengths-based and rooted in harm reduction and evidence-based practice. Within the Primary Health Care team, this RN will assist in the delivery of primary care services to vulnerable populations through education, individual support, screening, and outreach.

Requirements

  • RN with current College of Nurses of Ontario registration.
  • Minimum 3 years nursing experience, ideally in a combination of hospital, community and/or public health settings.
  • Addictions nursing experience required.
  • Ability to work flexible hours including evenings and weekends as required.
  • Strong organizational, communication and leadership skills.
  • Ability to work both independently, and collaboratively, within an interprofessional team environment.
  • Knowledge and proficiency in current evidence based practices of primary care delivery, with an emphasis on health promotion and harm reduction.
  • Awareness of and sensitivity to health issues particularly within individuals who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured).

Responsibilities

  • Provide nursing services within NorWest CHC’s Primary Care and the RAAM team.
  • Provide low-barrier addictions care to clients and their families within the RAAM clinic, using a strengths-based approach rooted in harm reduction and evidence-based practice.
  • Assist in the delivery of primary care services to vulnerable populations through education, individual support, screening, and outreach within the Primary Health Care team.
  • Perform clinical activities according to the College of Nurses standards for nursing practice.
  • Utilize professional assessment skills to identify physical, mental, and psychosocial needs of clients and families.
  • Respond to fluctuating workload and changing environment.
