Organization: VPH PTU Job Summary: Applies the nursing process to plan and implement the care of patients, including patient and family education and continuum of care planning. Effectively communicates pertinent patient/family information to the health care team. Collaborates in establishing patient goals and evaluates progress to ensure effective outcomes. Furthers professional growth by engaging in learning activities according to the Board of Nursing standards. This is a fully work from home position. Must Be local to Nashville for orientation and occasional on-site meetings. This is a PRN position. The Access RN with our Vanderbilt Psychiatric Hospital nursing team will use clinical judgment to determine patient placement and determine appropriate referrals for the inpatient setting by going through patient charts. The ideal candidate will have excellent communication skills to work on a multidisciplinary team of physicians, nurses, registration staff and work closely with emergency departments at some of our other hospitals such as Wilson County and Tullahoma.

Requirements

  • LIC-Registered Nurse - Licensure-Others
  • Relevant Work Experience
  • Graduate of an approved discipline specific program
  • Must be local to Nashville for orientation and occasional on-site meetings.

Responsibilities

  • Collaborates in establishing the plan of care, coordinates and implements care delivery while anticipating patient care needs across the continuum.
  • Analyzes comprehensive information pertinent to the patient's care and management to determine nursing diagnosis.
  • Continually utilizes the nursing process to evaluate patient's progress towards goals and applies appropriate interventions to ensure effective outcomes.
  • Employs strategies to promote health and a safe and healthy environment.
  • Uses clinical judgment to determine patient placement and appropriate referrals for the inpatient setting by reviewing patient charts.
  • Works on a multidisciplinary team of physicians, nurses, and registration staff.
  • Works closely with emergency departments at other hospitals.
