The Behavioral Health Clinical Navigator Registered Nurse is responsible for reviewing, processing, and ensuring the accuracy of clinical documentation. The Clinical Navigator is responsible for suggesting and implementing changes, educating field staff on documentation requirements, and ensuring state and federal requirements are consistently followed. The Behavioral Health Clinical Navigator RN optimizes quality of care across the clinical departments by undertaking efforts to review clinical competency and ensure regulatory adherence. Requirements

  • High School Diploma or GED
  • Associate degree in a Nursing related field required
  • 2 years of experience as a nurse in an appropriate clinical care setting
  • Home Care Home Base knowledge and platform familiarity with PointCare
  • Working knowledge of Federal and State rules and regulations for Medicare and Medicaid
  • Valid driver’s license and insurance
  • Reliable transportation to perform job tasks

Nice To Haves

  • Experience in management
  • Experience in Infection control
  • Experience in Home Health and Hospice surveys is preferred
  • Experience in a home health or hospice setting is preferred

Responsibilities

  • Reviewing, processing, and ensuring the accuracy of clinical documentation.
  • Suggesting and implementing changes to clinical documentation processes.
  • Educating field staff on documentation requirements.
  • Ensuring state and federal requirements are consistently followed.
  • Optimizing quality of care across clinical departments by reviewing clinical competency and ensuring regulatory adherence.
  • Training in a local CT branch.
