Work together to meet the needs of patients and families, to promote healing, and to improve quality of life. Collaborate as a key resource for patients, staff, and providers for wound, ostomy, continence, and skin care. This position will focus primarily on the urological and fecal ostomy patient population. Establish goals and strategies for meeting care coordination needs across settings with the patient, family, and/or other care providers. Assist a team of specialists treating many types of chronic and hard-to-heal wounds as needed. Conditions may include complex wounds, non-healing surgical wounds, pressure injuries and ulcers, traumatic wounds, wounds associated with leg swelling or related to slow blood flow to lower extremities. Utilize advanced wound treatments such as advanced tissue products, compression wraps, laser scar treatment, specialized dressings, surgical debridement, vacuum-assisted wound closure. Work on a great team that serves patients across multiple services throughout the hospital and/or ambulatory setting.
