We are seeking a compassionate and skilled Registered Nurse (RN) who is passionate about delivering high-quality patient care in the home setting. This is a Part-Time/ Per-Diem position with flexible scheduling available. The role involves conducting Start of Care (SOC) visits, performing skilled nursing visits and ongoing reassessments, providing hands-on skilled nursing care, delivering patient and caregiver education, collaborating with physicians and interdisciplinary team members, maintaining accurate clinical documentation, and supervising/delegating care to LPNs and Home Health Aides.

Requirements

  • Active Registered Nurse (RN) license in the state of practice
  • Strong clinical assessment and critical thinking skills
  • Knowledge of home health regulations and documentation standards
  • Excellent communication and interpersonal skills
  • Ability to work independently and manage time effectively
  • A passion for providing compassionate, patient-centered care

Nice To Haves

  • Experience in home health, skilled nursing, or a related healthcare setting preferred

Responsibilities

  • Conduct Start of Care (SOC) visits to complete comprehensive clinical assessments and develop individualized, physician-directed care plans
  • Perform skilled nursing visits and ongoing reassessments to monitor patient condition and adjust care plans as needed
  • Provide hands-on skilled nursing care, including wound care, medication management, injections, and chronic disease management
  • Deliver patient and caregiver education on diagnoses, medications, and preventive care to reduce hospitalizations
  • Collaborate with physicians and interdisciplinary team members to ensure coordinated, high-quality care
  • Maintain accurate and timely clinical documentation in compliance with state regulations and agency standards
  • Supervise and delegate care to LPNs and Home Health Aides to ensure quality and adherence to the plan of care
