This is a Part-Time/ Per-Diem position with flexible scheduling available. The role involves conducting Start of Care (SOC) visits, performing skilled nursing visits and ongoing reassessments, providing hands-on skilled nursing care, delivering patient and caregiver education, collaborating with physicians and interdisciplinary team members, maintaining accurate clinical documentation, and supervising/delegating care to LPNs and Home Health Aides.

Requirements

Active Registered Nurse (RN) license in the state of practice
Strong clinical assessment and critical thinking skills
Knowledge of home health regulations and documentation standards
Excellent communication and interpersonal skills
Ability to work independently and manage time effectively
A passion for providing compassionate, patient-centered care

Nice To Haves

Experience in home health, skilled nursing, or a related healthcare setting preferred

Responsibilities

Conduct Start of Care (SOC) visits to complete comprehensive clinical assessments and develop individualized, physician-directed care plans
Perform skilled nursing visits and ongoing reassessments to monitor patient condition and adjust care plans as needed
Provide hands-on skilled nursing care, including wound care, medication management, injections, and chronic disease management
Deliver patient and caregiver education on diagnoses, medications, and preventive care to reduce hospitalizations
Collaborate with physicians and interdisciplinary team members to ensure coordinated, high-quality care
Maintain accurate and timely clinical documentation in compliance with state regulations and agency standards
Supervise and delegate care to LPNs and Home Health Aides to ensure quality and adherence to the plan of care
