In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, this role works collaboratively as a member of an inter-professional team to provide direct patient care to clients of the Urgent Primary Care Centre (UPCC) or Primary Care Clinic. The position offers both timely and longitudinal primary care to unattached and attached vulnerable clients. The role involves initial client consultation, assessment, coaching, teaching, health promotion, intervention, client care services, and follow-up based on client needs and urgency. It ensures clients, families, and caregivers have access to timely, person-centred, continuous, and culturally safe quality health services. The role also facilitates and manages client transitions across the healthcare continuum to optimize recovery and adapts to changes in the client's condition to minimize avoidable admissions. Collaboration with acute, primary, and community care providers, including Nurse Practitioners, Physicians, specialists, and family supports, is crucial for client care planning and facilitating information flow across various settings. Care is provided through a client and family-centered, trauma-informed approach and evidence-informed practice. The role focuses on establishing dignifying, purposeful, and trusting relationships with clients, promoting self-determination and independence. It encourages knowledge exchange and empowers clients and their supports to engage within their communities, ensuring they are supported in navigating the healthcare system.
