About The Position

This role involves leading the execution of personalized care plans, monitoring and communicating patient milestones, coordinating the multidisciplinary care team, executing physician and nursing orders, and championing the discharge planning process to ensure a safe transition for patients and their families back home.

Requirements

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Registered Nurse credentialed from the Oklahoma Board of Nursing required.
Diploma from an accredited school/college of nursing OR professional licensure at time of hire required.

Coordinate the multidisciplinary care team to ensure seamless collaboration across all specialties, providing a unified and holistic healing experience for the patient.
Execute physician and nursing orders with clinical excellence, maintaining the highest standards of safety and accuracy in every treatment provided.
Champion the discharge planning process by collaborating with the healthcare team to ensure patients and their families have a clear, confident, and safe transition back to their homes.
