Requirements
- Graduate of an accredited school of nursing (Diploma, AD, BS, BSN)
- 2 years ED experience
- RN In State of CT or Permit; Basic Life Support (BLS)
- Computer skills, strong service excellence telephone skills
Responsibilities
- Responsible for using EPIC reports to identify ED patients that were discharged .
- Responsible for calling discharged patients to follow up on their visit and provide any necessary clarification of discharge instructions.
- Provides service recovery as needed.
- Calls to be made within 48 hours of patient discharge.
