The Registered Nurse Post Acute Network Skilled Nursing Facility Program Manager is responsible for serving as the liaison between the hospital and the post-acute network (PAN) skilled nursing facilities. This role manages and facilitates communication between inpatient care managers, external care providers, and outpatient care managers for high-risk patients transitioning in and out of the acute care setting and post-acute programs. The position assists in the transition of high-risk patients to post-acute programs and eventually back to their primary care physician (PCP) and community. The Program Manager monitors, collects, and analyzes data to evaluate the effectiveness, safety, and quality of patient care in the post-acute care network programs. They address questions and concerns from providers, patients, and families regarding post-acute programs in coordination with other care teams, acting as a conduit for problem-solving, resolution, and service recovery. This role ensures accurate placement of patients into the Post Acute Network program based on established criteria and provides Utilization Review for specific populations needing additional oversight. The position involves interaction with various hospital administrative teams, leaders, and directors, as well as post-acute providers at skilled sites. There will be some contact with patients and families regarding post-acute program and care options when service recovery or quality issues arise. The role involves monitoring appropriate post-acute programs, problem-solving unique situations, and optimizing data collection and information transfer across sites. A key responsibility is finding effective ways to manage and coach Sub-Acute Skilled Nursing Facility providers to shorten length of stay, reduce readmissions, and increase utilization of Advocate Aurora providers. The Program Manager evaluates resource utilization, analyzes sentinel events, and follows up on quality issues. They address issues of non-compliance with staff and MDs regarding patient referrals to appropriate programs and serious quality issues or quality of care trends related to post-acute providers. Recommendations for policies and procedures to enhance service delivery are also part of this role. The position is responsible for relationships within their assigned territory, including assigned hospitals, clinics, and skilled nursing facilities, as well as other post-acute providers like physician offices, Home Health Providers, and Assisted Living providers. Coverage for other positions may be required on a short-term or permanent basis. The role involves regular travel to meetings with Skilled Nursing Facility Staff and provides support to the clinical team assigned to the facility, removing operational barriers. The Program Manager is responsible for understanding and adhering to the organization's Code of Ethical Conduct and ensuring compliance with applicable policies, regulations, and laws.
